Nexus Health Resources Cuts Select Orange Regional Medical Center Patient Readmissions by Half

Reduction in readmissions means healthier patients

Middletown, NY – (June 12, 2014) – By stepping up to fill a new, critical gap in the healthcare industry, Nexus Health Resources – a Middletown-based healthcare coordination company founded by CEO and President Dr. Virginia Feldman – has reduced readmission rates for monitored patients at Orange Regional Medical Center (ORMC) by half.

Created to aid patients and hospitals in a rapidly-changing healthcare industry, Nexus Health provides transitional care, coordination and education to patients leaving the hospital and returning home. Nexus Health’s mission dovetails with the Affordable Care Act legislation which reduces payments to medical facilities that have excess readmissions within a 30-day window.

Nexus Health’s partnership with ORMC – formed earlier this year – has decreased readmission by half the previous rate, before being monitored, according to ORMC internal data. To accomplish the decrease, Nexus Health’s Care Coordinators have worked alongside discharged patients with congestive heart failure or pneumonia to schedule doctor’s appointments, provide medications to bedside and work collaboratively with clinical and quality staff. Nexus Health’s efficient processes, such as follow-up care, ensure there are no loose ends.
